For instructor candidates.
These are the working forms I use when I run Instructor Development workshops. They follow the PADI evaluation rubrics line for line, and they ask for one thing the paper forms do not: for every element you tick, when you heard it and what was said. A score without that record teaches nobody anything.
Fill one in while a fellow candidate presents, then send it. The PDF reaches me and a copy downloads for you.

The five-section PADI Knowledge Development rubric — introduction, objectives and scope, sequence, summary, handling and training aids — scored live as you tick what you observed, with a when-and-what note beside every element.

List your students and print a starting-weight grid for each one — by body type and exposure suit, for the pool and the quarry — plus a record sheet for the lead they actually used. Student weights stay in your browser; nothing is sent anywhere.
